Form Mail Script is a PHP formmailer script that lets you put a contact form on your Web site. Both HTML layout and email layout are template driven and customizable. This script includes several security features to prevent abuse by spammers.

Changes: This version allows the form data to be sent as CSV or Vcard attached to the email message. It also supports UTF-8 encoded email and the Return-path header. Another new feature allows it to compare form input.
