PHP PDFTemplate
PDFTemplate is a PHP 4 class that generates multi- page reports in PDF using an XML formatted input file with substitution support. It requires the pdflib plugin for PHP and PHP >=4.0.5.

Reguires you to setup an account to access, and that account does get spammed periodically, as I've received significant numbers of unwanted messages from phpclasses.
Further, the history of open source and freshmeat listing has been to avoid spam / account creation etc for access to LGPL code. Sad to see folks going that route, there are plenty of free places to host open source software.
